RAI / MDS Coordinator

This is an exceptional opportunity for an energetic, enthusiastic RN or RPN (current member in good standing with the College of Nurses of Ontario) with a minimum of three years in clinical, project leadership or managerial experience in Long Term Care. The successful candidate possesses a sound background in LTC as well as sound knowledge of the Long Term Care Act, Ministry of Health Long Term Care Program Standards, including documentation requirements.

The Resident Assessment Instrument (RAI) Coordinator is responsible for the overall administration, organization, education, co-ordination and implementation of the Minimum Data Set (MDS) for the interdisciplinary team. The incumbent will provide oversee the development, co-ordination and file retention for the data submission process to the Canadian Institute for Health Information (CIHI). 

You are a self-motivated individual who possesses effective communication, analytical, and assessment skills with strong problem solving and troubleshooting abilities. 

Practicing our corporate values of R.E.S.P.E.C.T., the successful candidate’s responsibilities will include, but are not limited to, the following:

  • Providing RAI MDS education and relevant software training, to members of the home interdisciplinary team using a train-the-trainer model. Utilizing AIS on-line training to support and assist with MDS education in the home;
  • Monitoring and providing on-site support to all MDS users in the home;
  • Developing and maintaining an up-to-date resident assessment schedule, ensuring accessibility to all users. Monitoring and auditing completion of MDS assessments to ensure assessments are completed on time and that all MDS timelines are met and adhered to;
  • Assisting MDS users in coding assessments - providing materials, assistance and support to ensure coding accuracy;
  • Working with RAI users and management team to develop processes that ensure data accuracy, efficient data collection processes, assessment completion and data utilization. Completing  MDS data audits to confirm accuracy of coding on a quarterly basis. Retaining results on file; where required developing action plans to address coding issues identified through the audit;
  • Reviewing, preparing, and submitting RAI data to the CIHI on a quarterly basis;
  • Generating RAI-MDS output reports and reviews with the management team and front line staff, facilitating their use in clinical and operational decision-making and quality improvement;
  • Assisting with the development of an auditing process related to data quality;
  • Ensuring RAI backup coordinator is current with all systems and processes to ensure sustainability of RAI MDS;
  • Actively promoting, supporting and assisting in customer service programs. Acting as a role model and promoting collaboration and coordination of the interdisciplinary team;
  • Representing Long Term Care Home at appropriate seminars, workshops and other meetings;
  • Ensuring MDS assessments, resident assessment protocols and care planning are completed within mandated timelines.
  • Responsible for other related duties as assigned.
  • The successful candidate must provide a current criminal record check and vulnerable sector search.
